Patients may be seen for a pain problem as a consultation from their primary physician or may self-refer to Arizona Pain Specialists. Appointments may be made by called during our regular office hours. Many insurance plans require pre-approval and referral to Arizona Pain Specialists. If this requirement applies to your plan, please have the referral ready at the time you make the appointment.
Medical records, diagnostic tests and x-rays related to your current problem are helpful in assisting the physician to diagnose and treat your condition. If these are available, please bring them with you to the appointment.
Forms for new patients
We have placed our forms for our new patients on the website. These forms include a Patient Intake Form which tells us about your pain problem and your medical history, the Patient Insurance Information form which describes your current health insurance coverage, and our Financial Policy & Waiver form which describes the financial policies of Arizona Pain Specialists. We encourage you to download these files and read them thoroughly.
